DO YOU have an NDIS pack­age and need sup­port?

Rochester and El­more Dis­trict Health Ser­vice is a reg­is­tered provider for NDIS ser­vices and sup­ports and can as­sist peo­ple liv­ing in Rochester, El­more and sur­round­ing ar­eas.

REDHS is able to work with par­tic­i­pants to achieve their goals and de­liver sup­ports in NDIS plans in the fol­low­ing ar­eas: im­proved health and well­be­ing; as­sis­tance with so­cial and com­mu­nity par­tic­i­pa­tion; and as­sis­tance with daily life.

Ser­vices avail­able in­clude com­mu­nity nurs­ing care, ex­er­cise phys­i­ol­ogy, group and cen­tre based ac­tiv­i­ties, prod­ucts and equip­ment, home mod­i­fi­ca­tions, house­hold tasks, per­sonal ac­tiv­i­ties, di­etet­ics, oc­cu­pa­tional ther­apy, phys­io­ther­apy, po­di­a­try and so­cial work.

‘‘REDHS staff are ex­pe­ri­enced in de­liv­er­ing ser­vices to peo­ple liv­ing with a dis­abil­ity, and con­tinue to pro­vide sup­port for younger peo­ple with dis­abil­i­ties un­der the Home and Com­mu­nity Care program for younger peo­ple (HACC – PYP),’’ a REDHS spokesper­son said.

‘‘Par­tic­i­pants can be as­sured the ser­vices and sup­ports of­fered and de­liv­ered by REDHS are in ac­cor­dance with NDIS terms of business for reg­is­tered sup­port providers and the fees out­lined in the NDIS price guide.

‘‘To ac­cess NDIS sup­ports from REDHS, we sug­gest par­tic­i­pants ar­range a meet­ing with a staff mem­ber.’’
