THOSE at risk of, or with, di­a­betes at Monto can see a doc­tor at the Monto Fam­ily Prac­tice for di­ag­no­sis, on­go­ing care, preven­tion and also screening of fam­ily his­to­ries of the con­di­tion.

The prac­tice also runs chronic dis­ease man­age­ment based on a plan that guides pa­tients through, and ed­u­cates them about, the ill­ness.

The Monto Fam­ily Prac­tice has a pathol­ogy ser­vice with all blood test­ing fa­cil­i­ties and the prac­tice has ac­cess to di­a­betes ed­u­ca­tion, foot care (po­di­a­try), di­eti­cian ser­vices, en­docrinol­ogy and oph­thal­mol­ogy.

These ser­vices are es­sen­tial for di­a­betic care as the con­di­tion di­rectly af­fects the blood ves­sels of the eye (di­a­betic retinopa­thy), kid­ney (di­a­betic nephropa­thy) and nerves (di­a­betic neu­ropa­thy).
