MANY peo­ple feel fool­ish ev­ery April when they get hit yet again by above-in­fla­tion health fund pre­mium rises. A fam­ily pay­ing $2000 in 2002 would now have to shell out $4840 for the same pol­icy. Funds cop the flak, but what about hospi­tals and doc­tors? Med­i­cal costs and the num­ber of ex­pen­sive pro­ce­dures have ex­ploded. A Four Cor­ners pro­gram found knee re­place­ments cost the health sys­tem more than $1 bil­lion a year and a fifth were un­nec­es­sary.
