CHRONIC DISEASE MANAGEMENT
THE Chronic Disease Management (CDM) is a fully bulk billed program by Medicare.
It enables GPs to plan and coordinate the health care of patients with chronic or terminal medical conditions.
A chronic medical condition is one that has been or is likely to persist for six months or longer, for example asthma, cardiovascular disease, diabetes, musculoskeletal conditions, cancer and stroke.
Patients who qualify for the CDM are eligible for Medicare rebates for certain allied health services such as visits to the podiatrist, dietician, physiotherapist or occupational therapist.
