THE Chronic Dis­ease Man­age­ment (CDM) is a fully bulk billed pro­gram by Medi­care.

It en­ables GPs to plan and co­or­di­nate the health care of pa­tients with chronic or ter­mi­nal med­i­cal con­di­tions.

A chronic med­i­cal con­di­tion is one that has been or is likely to per­sist for six months or longer, for ex­am­ple asthma, car­dio­vas­cu­lar dis­ease, di­a­betes, mus­cu­loskele­tal con­di­tions, can­cer and stroke.

Pa­tients who qual­ify for the CDM are el­i­gi­ble for Medi­care re­bates for cer­tain al­lied health ser­vices such as vis­its to the po­di­a­trist, di­eti­cian, phys­io­ther­a­pist or oc­cu­pa­tional ther­a­pist.
