Two GPs and a skin clinic have breached the health code for oversights relating to a diagnosis of skin cancer. A man sought treatment for a lump on his neck, but his GP overlooked referring him for surgery. He then visited a doctor at a skin clinic, who didn’t communicat­e effectivel­y with his GP, or give a follow-up plan. There was then a delay in the skin clinic sending consultati­on notes to the man’s GP. He later died, after receiving surgery and radiation therapy for a type of cancer that had spread to his lymph nodes.
