Pa­tient’s fa­tal de­liv­ery

A BLIND woman died after a de­liv­ery driver mis­tak­enly gave her med­i­ca­tion meant for a neigh­bour with a sim­i­lar name, a Bri­tish in­quest has heard.

Christina Ann Fletcher, 58, was given 60 mor­phine tablets in­tended for Ann Fletcher, an Alzheimer’s pa­tient who lived four doors away.

More than a week later, when the phar­macy that sent the or­der re­alised what had hap­pened, po­lice found the for­mer nurse dead at her home in Hey­wood, near Manch­ester, with nine of the mor­phine tablets miss­ing.

Christina Fletcher, who was al­most com­pletely blind, was also treated for epilepsy, de­pres­sion and anx­i­ety.

De­liv­ery driver An­drew Watkins said Christina Fletcher was some­times un­able to sign for her pre­scrip­tions. On Au­gust 4 last year he had not asked her to sign and had “de­liv­ered the wrong pre­scrip­tion by mis­take”. The in­quest con­tin­ues. – Daily Mail
