Q I’m suf­fer­ing from acne, acne scars and large pores. I’ve cut down on sugar, dairy and oily foods and now in­clude a lot of greens in my diet. A doc­tor has sug­gested laser treat­ment for acne and large pores. What is your opin­ion?

AThough you haven’t men­tioned your age, I as­sume that you are in your late teens or early 20s. Your is­sues will need to be ad­dressed in­di­vid­u­ally through a de­tailed ex­am­i­na­tion. For in­stance, acne can be treated with var­i­ous op­tions rang­ing from top­i­cal creams, gels, so­lu­tions and cleans­ing masks as a first-line treat­ment. As a sec­ond-line treat­ment, oral an­tibi­otics can be pre­scribed depend­ing upon the sever­ity of the acne and re­spon­sive­ness.

As a third-line treat­ment, a course of med­i­ca­tion to treat se­vere and re­sis­tant acne can be pre­scribed.

Acne in gen­eral is di­ag­nosed only on clin­i­cal ex­am­i­na­tion. How­ever, in se­vere cases, par­tic­u­larly in fe­male pa­tients, cer­tain in­ves­ti­ga­tions could be worth per­form­ing.

Serum hor­monal pro­files and pelvic ul­tra­sounds are among the nec­es­sary tests to rule out any un­der­ly­ing con­di­tions, such as Poly­cys­tic Ovary Syn­drome (PCOS.) In these cases an en­docrino­log­i­cal con­sul­ta­tion is highly rec­om­mended.

Acne scars are treated based on their clin­i­cal as­sess­ment, whether they are hy­per­trophic (papu­lon­odu­lar scars) or at­rophic (ice-pick scars.)

Com­mon treat­ment op­tions in­clude var­i­ous acid peels as well as use of lasers. Here again, I would strongly em­pha­sise that you select a skilled der­ma­tol­o­gist with a good rep­u­ta­tion to get the best pos­si­ble re­sults

The prob­lem of your large pores is a sis­ter is­sue re­lated to your acne, and in­di­cates the pres­ence of hy­per­ac­tive oil (se­ba­ceous) glands in the der­mal layer of your skin.

With the im­prove­ment of acne, par­tic­u­larly through oil con­trol­ling treat­ments, these large pores tend to im­prove to a great ex­tent. In cases of in­com­plete suc­cess, top­i­cal gen­tle fa­cial peels or mi­cro­der­mabra­sion treat­ment ses­sions can com­plete the rest of the job.
