Personnel files were kept poorly

The review was particular­ly critical of the trust’s record keeping.

When a member of staff attempted to carry out an internal review of alleged offences by Bundhun they said his personnel file was “a bit all over the shop”.

Staff said warnings had been removed from his file because the practice had been to destroy records after a year had passed.

Verita said it had been clear informatio­n was missing, such as disciplina­ry outcomes. It added: “Due to the trust’s poor record-keeping practices, we were unable to perform analysis at the desired level of detail.

“When we received his files, they were neither arranged chronologi­cally nor systematic­ally and material was missing.”

It said line managers should have been responsibl­e for the files and criticised the “central trust team” for not making sure that was happening.

“The lack of central oversight and scrutiny of these processes restricted the trust’s ability to recognise the risks Vijay Bundhun posed.

“The trust lacked an overarchin­g narrative on the incidents of alleged misconduct against Vijay Bundhun.”
