HHS needs to make “mean­ing­ful use” crys­tal clear and con­crete, oth­er­wise small physi­cian prac­tices will take their usual stance on new tech­nol­ogy and “wait” (“ ‘Mean­ing­ful use’ at last,” Jan. 4, p. 4).

It seems to me HHS should make the orig­i­nal buy-in in 2011 eas­ily achiev­able, such as re­quir­ing only a cer­ti­fied elec­tronic health record and use of e-pre­scrib­ing. Then, in later years they can add more cri­te­ria—which will give them more time to cre­ate test sites and make tan­gi­ble the more dif­fi­cult tasks of in­ter­change and qual­ity mea­sure­ment. Make it too hard to un­der­stand or dif­fi­cult to achieve and the whole ef­fort will be coun­ter­pro­duc­tive.
