Why can we not move to the value-based pur­chas­ing, ac­count­able care model more quickly (“Bill would boost doc pay, hit hos­pi­tals,” ModernHealth­care.com, May 20)? It is im­por­tant to have physi­cian and hos­pi­tal qual­ity/out­comes and fi­nances in align­ment. Or­ga­ni­za­tions such as Geisinger Health Sys­tem have set a great ex­am­ple on how to achieve high qual­ity of care and con­trol es­ca­lat­ing ex­penses.

Pri­vate pay­ers are work­ing more quickly in this di­rec­tion. Hos­pi­tals dealt with the fall­out of the Medi­care Mod­ern­iza­tion Act and the av­er­age sales price re­im­burse­ment model in the out­pa­tient set­ting and the move­ment of pa­tients from the physi­cian of­fice to the hos­pi­tal out­pa­tient set­ting.

We need to de­velop a model that works for all par­ties, but keep­ing in mind that we pro­vide care to pa­tients and they are the No. 1 pri­or­ity.
