I am a homecare nurse who sees patients stuck in a revolving door between hospitalization with observation status, home, and then back into the hospital on observation status because they were sent home rather than to the appropriate skilled-nursing facility for intense rehab.
We can only improve outcomes and prevent re-hospitalizations with an adequate level of resources, including teaching provided to patients at crucial points in their illnesses.
Counting observation days toward the three-day rule would be huge in improving patient safety.
