In your cov­er­age of newly pub­lished find­ings on ad­verse events and the strength of the In­sti­tute for Health­care Im­prove­ment’s Global Trig­ger Tool for de­tect­ing them (“The more you look,” April 11, p. 8) there is a fac­tual er­ror. You write that the GTT re­quires “the use of trained re­view­ers from out­side the or­ga­ni­za­tion,” which is not the case.

The tool was de­signed for use by re­view­ers from within the hos­pi­tal, and there is no ex­pec­ta­tion or re­quire­ment for ex­ter­nal re­view­ers. As a co-de­vel­oper of the tool and co-au­thor of the lat­est study, I ap­pre­ci­ate the op­por­tu­nity to cor­rect this im­por­tant point. Fran Grif­fin
