Re: “Se­nate OKS two-month freeze on doc pay,” Modern­health­care.com, Dec. 20: Two months? Whoop de doo! I am with Amer­i­can Med­i­cal As­so­ci­a­tion Pres­i­dent Dr. Peter Carmel on this one. “Wait­ing un­til the fi­nal week of the leg­isla­tive ses­sion to ad­dress an is­sue Congress knew about all year is no way to con­duct busi­ness for the coun­try.”

Whose idea of me­dia and po­lit­i­cal ma­nip­u­la­tion is this? When we look at how many post­ings about this on all the health­care rags and chan­nels, there has to be some­one be­hind the strat­egy to act this way and show the health­care in­dus­try just how much we (don’t) value them and as­sume they will still be able to sus­tain­ably be open for busi­ness.

Many of my clients have asked me what to do. Many doc­tors’ of­fice staff leave their jobs right af­ter the of­fice Christ­mas party when bonuses are handed out and it’s time for new health­care de­ductibles on a health ben­e­fit plan. The docs are call­ing me and ask­ing, “Should I re­place the per­son who left? I can’t sus­tain the busi­ness if they cut our pay. I am al­ready at about 83% over­head to gross.”

If that hap­pens large-scale, the rift will rip­ple to un­em­ploy­ment, sti­fling job growth and mak­ing the case man­age­ment and other aspects of pa­tient-cen­tered med­i­cal home and ac­count­able care or­ga­ni­za­tions fan­tasy.

As we say in the South, “All y’all that think we can do ACO and PCMH in the face of this pay here cut, I’ve got dis­count tick­ets for you to meet me at the Dumbo the Ele­phant ride in Fan­ta­sy­land at Dis­ney World.”
