Re: “NAPH presses states on Med­i­caid ex­pan­sion,” Mod­ern­Health­, July 12: I think the elected lead­ers not want­ing to ex­pand Med­i­caid in their states are con­cerned about how they are go­ing to pay for the ex­panded pro­gram.

Dr. Bruce Siegel is right to want to have those peo­ple cov­ered, but the eco­nomic re­al­i­ties have to be con­sid­ered. With many states al­ready try­ing to cover huge bud­get short­falls, adding 13 mil­lion to the Med­i­caid rolls isn’t eco­nom­i­cally sus­tain­able. An­other is­sue they will have to deal with is to de­ter­mine if there are enough physi­cians will­ing to take Med­i­caid pay­ments to pro­vide care; I think that may turn out to be a big­ger prob­lem all across the coun­try.
