The ar­ti­cle “Hos­pi­tal re­call man­age­ment crit­i­cized” in the Nov. 24 is­sue (p. 12), should have said that Trin­ity Health up­dated its med­i­cal-de­vice re­call man­age­ment pro­gram in 2008.

A sur­vey of health­care CEOs found that about 4 out of 10 had en­gaged their boards in suc­ces­sion plan­ning. An edit­ing er­ror in the Nov. 24 Guest Ex­pert Com­ment col­umn (p. 25) led to an in­cor­rect state­ment of the ra­tio.
