Applies to services performed by an out-of-network provider at a network facility or when a network doctor refers to a non-network provider

Patients do not have to pay out-of-network charges for emergency services that are higher than in-network charges

Out-of-network providers may dispute the amount the health plan pays them for emergency services through an independent dispute resolution process
