Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Line prep table is not holding food at 41 degrees or below; food checked was 60 to 63 degrees; a service call has been made, but facility was still using the unit without an alternate way to keep food cold or use time as a control temporarily; food was destroyed; facility will use small batches of food and not hold anything more than four hours; unit will only be used for guacamole, sour cream, pico, cheese and lettuce; all other food will be stored in another refrigerator. Fajita cooler is not holding food at 41 degrees or below; food destroyed as needed and a limited amount of food will be stored in the unit to work out of.
Noncritical violations: Cut, raw meat stored in to-go type bags in chest freezer; do not store food in direct contact with items not meant for food. Unwrapped sliced cheese stored on a tray in the walk-in; tray is not clean and food destroyed. Shelves in walk-in cooler have food and other debris on them; clean all shelves and maintain them in a clean condition. Bar keg cooler gasket and frame not clean; possible mold growing on gasket and frame. The gasket on fajita prep table is not in good condition. 1889 Bella Vista Way, Bella Vista Critical violations: There was unauthorized personal in the kitchen. Mashed potatoes were 119 degrees when they should be 170 degrees. Mashed potatoes in the hot hold were less than 175 degrees. There was no prepared sanitizer on site at inspection.
Noncritical violations: Wiping cloths shall be used as intended and in compliance with regulations pertaining to retail food establishment and must be used without sanitizer residue.

The Creeks Golf Course
1499 S. Main St., Cave Springs Critical violations: Employee came in from outside and scooped ice into a cup, but did not wash hands first; if ice is going to be used in beverages, hand washing before scooping is required. Discontinue sale and use of all foods from unapproved sources; personal food cannot be used by or stored in the facility kitchen; all food must be prepared in the permitted kitchen; no food can be prepared in a private home and be used in the facility. Employee did not know to sanitize utensils/equipment after washing so items have not been sanitized for an unknown amount of time. Brauts on rollers are at 125 degrees; the unit was adjusted to hold food at 135 degrees or above; according to employee, items have been cooked completely and had been placed on rollers about 30 minutes prior to the inspection. Packages of hot dogs in the white refrigerator are at 48 degrees; stored cold foods should be 41 degrees or lower; hot dogs destroyed as needed. Packages of hot dogs and lunch meat are not date marked as needed; repeat violation; date marking for ready to eat, potentially hazardous foods and time/temperature control for safety food prepared on site or opened commercial container held for more than 24 hours.
Noncritical violations: There were no test strips to check sanitizer concentration; repeat violation.
Gaby’s Bakery
1803 S. Eighth St., Suite 8, Rogers Critical violations: Tamales were at 127 degrees in a large pot by the cash register; hot foods must be held at an internal temperature of 135 degrees or above; reheat to 165 degrees and keep above 135 degrees.
Noncritical violations: The spray bottle does not have label indicating content on shelf in dough prep room.
Kennedy Coffee Retail

Walton Blvd, Bentonville Critical violations: Hand washing sink, aids and devices may not be used for food preparation or utensil washing, or a service sink or in any other manner other than handwashing. Sponges may not be used in contact with cleaned and sanitized or in use food contact surfaces. Wash ware was stored on wet absorbable towels.

Critical violations: Food in the twodoor upright from the breakfast is not at 41 degrees, items were moved to the walk in for proper cooling.

June 27 The Salvation Army

I St., Bentonville Critical violations: Food server was gloved without proper hand washing; must wash hands and time food contamination may have occurred such as when changing tasks and as described in the Arkansas Rules and Regulation Pertaining to Food Establishments section. There was no prepared sanitizer on site at the time of inspection. The 3C sink was used by patrons to fill beverage containers and rinse hands.
Noncritical violations: There was packaged foods stored below the hand washing sink; food must be stored in compliance with established regulations.

All food checked in the hot case was not at 135 degrees or above; case may have not been turned on and food was destroyed.

2806 S. Walton Blvd., Bentonville Critical violations: Wash hands any time contamination could have occurred, when changing tasks and as described in the Arkansas rules and regulations; the kitchen food handler put gloves on without washing hands. Must hot hold greens higher than 106 degrees.
Noncritical violations: Food must be stored in compliance with established regulations; there was a case of lemons in direct contact with cooler floor.
