1889 Bella Vista Way, Bella Vista Crit­i­cal vi­o­la­tions: There was unau­tho­rized per­sonal in the kitchen. Mashed pota­toes were 119 de­grees when they should be 170 de­grees. Mashed pota­toes in the hot hold were less than 175 de­grees. There was no pre­pared san­i­tizer on site at in­spec­tion.

Non­crit­i­cal vi­o­la­tions: Wip­ing cloths shall be used as in­tended and in com­pli­ance with reg­u­la­tions per­tain­ing to re­tail food es­tab­lish­ment and must be used with­out san­i­tizer residue.

Dis­con­tinue sale and use of all foods from un­ap­proved sources; per­sonal food can­not be used by or stored in the fa­cil­ity kitchen; all food must be pre­pared in the per­mit­ted kitchen; no food can be pre­pared in a pri­vate home and be used in the fa­cil­ity. Em­ployee did not know to san­i­tize uten­sils/equip­ment af­ter wash­ing so items have not been san­i­tized for an un­known amount of time. Brauts on rollers are at 125 de­grees; the unit was ad­justed to hold food at 135 de­grees or above; ac­cord­ing to em­ployee, items have been cooked com­pletely and had been placed on rollers about 30 min­utes prior to the in­spec­tion. Pack­ages of hot dogs in the white re­frig­er­a­tor are at 48 de­grees; stored cold foods should be 41 de­grees or lower; hot dogs de­stroyed as needed. Pack­ages of hot dogs and lunch meat are not date marked as needed; re­peat vi­o­la­tion; date mark­ing for ready to eat, po­ten­tially haz­ardous foods and time/tem­per­a­ture con­trol for safety food pre­pared on site or opened com­mer­cial con­tainer held for more than 24 hours.

3305 S.W. I St., Bentonville Crit­i­cal vi­o­la­tions: Food server was gloved with­out proper hand wash­ing; must wash hands and time food con­tam­i­na­tion may have oc­curred such as when chang­ing tasks and as de­scribed in the Arkansas Rules and Reg­u­la­tion Per­tain­ing to Food Es­tab­lish­ments sec­tion. There was no pre­pared san­i­tizer on site at the time of in­spec­tion. The 3C sink was used by pa­trons to fill bev­er­age con­tain­ers and rinse hands.

Non­crit­i­cal vi­o­la­tions: There was pack­aged foods stored be­low the hand wash­ing sink; food must be stored in com­pli­ance with es­tab­lished reg­u­la­tions.
