A June 6 article about problems in a federal investigation of a lethal defect in a General Motors automobile ignition switch incorrectly quoted U.S. Transportation Secretary Anthony Foxx as saying that “defective agencies, like defective people, need the capacity for self-reflection and to make room for selfimprovement.” Foxx said that “effective agencies, like effective people, need the capacity for self-reflection and to make room for selfimprovement.”
