HAMMONDCARE BEGAN in 1932 as Anglican Archdeacon Robert Hammond responded to the eviction of innercity families in Sydney during the Great Depression. Within five years the ‘pioneer settlement’ near Liverpool was home to 110 families. HammondCare then identified a new pressing need: the plight of the disadvantaged elderly. This led to the opening of one of the first integrated aged care homes in Australia. Today, as an independent Christian charity, HammondCare provides so much more, including residential, dementia and palliative care, as well as home care in metropolitan and rural regions across NSW, ACT, Victoria, and Queensland. To ensure home care clients feel valued and are given the highest level of care, HammondCare partners with clients and families through small, consistent care and clinical teams. HammondCare’s relationshipbased model of care means they get to know the client and their family as part of delivering and managing personalised services. This is backed by an Australian-based 24/7 Support Team and an SMS service for notifying of changes and updates. To ensure care teams are frequently updated with the right skills, HammondCare provides a range of education opportunities, including workplace training, and formal qualifications in Ageing Support. Care workers receive an intensive on-boarding programme, while Care Managers are provided with learning pathways to enhance case management skills. Support staff also participate in detailed customer service training. HammondCare has renowned specialist clinical expertise, especially in dementia and palliative care. Alongside its own service improvement research and dedicated quality, safety and risk team, HammondCare highly values feedback from clients and their families.
