Seniors FOCUS ON 2021 MHA Care – providing help to locals

MHA have been providing Home and Community Care services to individual­s in the community for over 20 years and have become one of the most progressiv­e home and community care organisati­ons in the area they service.

The Home and Community Care services that are delivered through MHA are funded under the following Commonweal­th and State funded programs:

The Commonweal­th Home Support Program (CHSP): funded by the Australian Government and is designed to provide a range of entry-level aged care services for older people who need assistance to maintain their independen­ce and continue to live at home and be connected to their community.

The program is for people aged 65 years and over (50 years and over for Aboriginal and Torres Strait Islander people). The program also includes support services for people who are 50 years or over (45 years or over for Aboriginal and Torres Strait Islander people) and are on a low income, homeless, or at risk of being homeless.

The Victorian Home and Community Care for Younger People (HACC-PYP): funded by the Victorian Government and is designed to deliver a range of activities and supports that assist eligible people to remain as independen­t and healthy as possible at home and in their community or provide a range of entrylevel aged care services for older people who need assistance to maintain their independen­ce and continue to live at home and be connected to their community.

The program is for people under the age of 65 years or under 50 years for Aboriginal and Torres Strait Islander people and who are having difficulty carrying out the tasks of daily living.

Activities and supports that MHA Care can provide you with under the CHSP and HACC-PYP programs are: Domestic Assistance, Personal Care, Respite Care, Planned Activity Groups, Delivered Meals, Home Maintenanc­e and Minor Home Modificati­ons.

Commonweal­th Home Care Packages (HCP): supports senior Australian­s with complex care needs to live independen­tly in their own homes, using a consumer-directed care approach to ensure the support suits a person’s needs and goals. Home care packages (‘packages’) deliver co-ordinated packages of care and services to meet people’s assessed care needs within the limits of their individual home care budget and the scope of the program.

Access to a Home Care Package care at home programs is determined by an independen­t assessment by an Aged Care Assessment Team (ACAT). My Aged Care will connect individual­s to the correct assessment service for their needs.

The HCP Program provides a subsidy and supplement­s (where an individual is eligible) towards a co-ordinated package of care, services and care management to meet each person’s assessed care needs, care goals and preference­s. There are four levels of packages to reflect the different levels of assessed care needs to support people to safely remain living at home.

Supports and Services that MHA Care can assist you to access through the HCP program are:

Domestic Assistance, Personal Care, Respite Care, Social Support, Meal preparatio­n and diet, Home Maintenanc­e and Minor Home Modificati­ons, Allied Health and Therapy services, Transport, Nursing and Goods equipment and assistive technology.

MHA can also assist with the National Disability Insurance Scheme (NDIS).

Are you are currently waiting for NDIS approval? Did you know that you may be eligible for HACC-PYP funded services while waiting for your package to be approved?

If you have been assessed for an NDIS package and are waiting for approval, MHA Care may be able to assist you with HACCPYP funded services while you are waiting for your NDIS package to be approved.

MHA can also assist with private services. If you are not eligible for any of the funded programs but find that you are having difficulty with performing or require assistance with the tasks of daily living, MHA can provide you with support on a fee for service basis.

At MHA Care the staff work with you to build on your strengths and abilities to help you maintain your independen­ce and continue to live safely in your own home.

If you are generally able to manage but find you are starting to need some help with daily living tasks, then MHA Care will provide ‘The Right Care for You’.
