The First Hospital Affiliated to The Fourth Military Medical University, Xi’an 710000, China Abstract: Objective To study the clinical efficacy of Sanhuang Decoction combined with liraglutide injection for the treatment of senile type 2 diabetes mellitus (T2DM) with obesity; To explore the possible mechanism of action. Methods According to the random number table method, 132 cases of elderly patients with T2DM complicated with obesity were divided into control group and study group, with 66 cases in each group. Patients in the control group were treated with liraglutide injection, once a day, the maximum injection <1.8 mg/d (changed every two weeks), subcutaneous injection; while patients in the study group were treated with Sanhuang Decoction on the basis of the control group, one dosage per day, taken in the morning and evening. Three months were one treatment course for each group. The clinical efficacy of the two groups was analyzed. The glucolipid metabolism indexes (fasting blood glucose, glycosylated hemoglobin, insulin resistance index, total cholesterol, triglyceride, apolipoprotein B), oxidative stress and inflammatory factors were observed before and after treatment. Adverse reactions of both groups were compared. Results The total effective rate was 90.91% (60/66) in the study group and 69.70% (46/66) in the control group, with statistical significance (P<0.05). Compared with before treatment, the levels of glucolipid metabolism

index, serum MDA, CAT, and plasma TNF-α, IL-6 and hs-CRP in both groups significantly decreased, while the levels of serum GSH⁃Px, SOD and plasma ADP significantly increased (P<0.05). After treatment, glucolipid metabolism, oxidative stress and inflammation factors of the study group were better than control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion Sanhuang Decoction combined with liraglutide injection can treat elderly patients with T2DM complicated with obesity through affectting glycolipid metabolism, oxidative stress and inflammatory response.
