a) Cov­er­ing let­ter from com­pany with nam­ing list, pass­port de­tails and re­la­tion with the com­pany of all ap­pli­cants. The let­ter should be duly signed by the au­tho­rised sig­na­tory and tak­ing the guar­an­tee of all ap­pli­cants that they will re­turn to In­dia on com­ple­tion of the tour. b) Cov­er­ing Let­ter from ap­pli­cant on busi­ness let­ter head (if self em­ployed) or on plain pa­per (if em­ployed) men­tion­ing name, des­ig­na­tion, pass­port num­ber, pur­pose and du­ra­tion of visit in brief. The let­ter should be duly signed by the ap­pli­cant and ad­dressed to–The Visa Of­fi­cer, Em­bassy of the Czech Re­pub­lic, New Delhi. Ap­pli­cant’s cur­rent ad­dress should also be men­tioned on Cov­er­ing Let­ter where ap­pli­cant wants pass­port to be de­liv­ered. c) A let­ter from the tour op­er­a­tor with nam­ing list and pass­port de­tails of all the ap­pli­cants. d) Com­pany’s (cor­po­rate) brief pro­file.

The let­ter should be ad­dressed to The Visa Of­fi­cer, Em­bassy of the Czech Re­pub­lic, New Delhi.
