More GPs join SDHB scheme

THE transition from doctors’ general practices to Health Care Homes has picked up pace, with five more entering the Southern District Health Board’s programme.

Health Care Homes are modernised GP practices, with an emphasis on increased use of technology and improving access to services.

In July, four practices took the plunge, and last week another five joined them: Waihopai Health Services (Invercargi­ll), Broadway Medical Centre (Dunedin), Junction Health (Cromwell) and Wanaka Medical Centre and Aspiring Medical Centre (both Wanaka).

Health Care Homes receive $12,500 for startup costs and can apply for up to $17,500 for other costs.

They receive $8 per enrolled patient per annum for three years, with an extra $8 per patient a year after reaching agreed targets.

Another seven practices are expected to begin becoming HCHs early next year and eventually the health board hopes all practices will adopt the model.

A key part of the SDHB and WellSouth’s primary and community care strategy, HCHs are intended to provide benefits including more preventive care, a reduction in emergency department presentati­ons, and allowing people requiring longterm care to stay in their homes longer.

WellSouth chief executive Ian Macara said feedback from the first HCHs had been encouragin­g.
