Cut the scalp and peel back the skin to expose the eye sockets, nose and cheekbones.

Cut out the upper facial bones, including the cheeks, upper teeth, nose and eye socket, so this section of the face is floating, separated from the skull and lower jaw.

Sew the scalp back up and screw frame or halo into the top of the skull, and attach with wire to bones in the nose. The wire is then wound forward in small amounts every day, bringing the middle of the face slowly forward.
