How it happened
February 26, 2016 Baby Royal is born. His parents Nasir Uddin and Nikita Winiata ( pictured) separate.
April Winiata and Baby Royal move into a friend’s home where Surender Mehrok is a boarder. He is wanted by police for assault on a child at the home in January 2016.
May 20 Uddin calls CYFS and makes a “Report of Concern” about Baby Royal after Winiata repeatedly calls him at 1am. A social worker should have visited the home within seven days of the report.
May 31 The home visit does not take place within a seven- day timeframe so a CYFS supervisor manually downgrades the urgency of the response. The case is given to a trainee social worker.
June 2 The trainee calls Uddin and Baby Royal’s grandmother in Whanganui. Supervisor does not check on progress and no home visit is scheduled. June 7 Baby Royal dies after fatal injuries inflicted by Mehrok.
