Cer­ti­fied copy of the child’s unabridged birth cer­tifi­cate. This ap­plies to sin­gle par­ents/guardians as well as a mom or dad trav­el­ling with­out the other par­ent. Cer­ti­fied copy of the child’s unabridged birth cer­tifi­cate. An af­fi­davit from the other par­ent or le­gal guardian, con­firm­ing their con­sent for the adult to travel with the child.

If a sin­gle par­ent has full parental re­spon­si­bil­i­ties and rights or le­gal guardian­ship of the child, he/she must pro­duce the court or­der to prove this.

If ap­pli­ca­ble, a death cer­tifi­cate of a de­ceased par­ent.

ADULT TRAV­EL­LING WITH A CHILD WHO IS NOT HIS/HER BIOLOGICAL CHILD

Cer­ti­fied copy of the child’s unabridged birth cer­tifi­cate. An af­fi­davit from the par­ents or le­gal guardians of the child, con­firm­ing their con­sent for the adult to travel with the child.

Cer­ti­fied copies of the iden­tity doc­u­ments or pass­ports of the par­ents or le­gal guardians of the child.

Con­tact de­tails of the par­ents or le­gal guardians of the child.

AN UN­AC­COM­PA­NIED MI­NOR

*In ad­di­tion to the above, chil­dren still need a valid pass­port and rel­e­vant visas to travel.

A cer­ti­fied let­ter of af­fi­davit from both the child’s par­ents (un­less one par­ent has full cus­tody) or le­gal guardian(s), giv­ing their con­sent for the child to travel into or out of South Africa.

If a sin­gle par­ent has full parental re­spon­si­bil­i­ties and rights or le­gal guardian­ship of the child and has writ­ten the let­ter/ af­fi­davit giv­ing con­sent, that par­ent must also pro­vide a cer­ti­fied copy of the court or­der as proof.

The con­tact de­tails of the par­ents or le­gal guardian of the child in the coun­try of ori­gin.

A let­ter from the per­son who is to re­ceive the child in the des­ti­na­tion coun­try, con­tain­ing his/her res­i­den­tial ad­dress and con­tact de­tails where the child will be stay­ing.

A copy of the iden­tity doc­u­ment or valid pass­port and visa or per­ma­nent res­i­dence per­mit of the per­son who is to re­ceive the child in the des­ti­na­tion coun­try.
