What exactly is personalised care and what can it provide to patients?
PERSONALISED care is a different way of working that has been brought in by the NHS to encourage patients to work in genuine partnership with the health and social care professionals looking after them.
Central to this new approach is putting what matters most to each individual patient at the heart of every conversation.
Doing things like this gives people more choice and control over the way their treatment is planned, allowing their voice to be heard and prioritising exactly what matters to each person, based on their individual circumstances, strengths and needs.
By providing ongoing non-judgemental, compassionate person-centred support and understanding, we aim to empower people to strive to become the very best version of themselves.
