What exactly is personalis­ed care and what can it provide to patients?

PERSONALIS­ED care is a different way of working that has been brought in by the NHS to encourage patients to work in genuine partnershi­p with the health and social care profession­als looking after them.

Central to this new approach is putting what matters most to each individual patient at the heart of every conversati­on.

Doing things like this gives people more choice and control over the way their treatment is planned, allowing their voice to be heard and prioritisi­ng exactly what matters to each person, based on their individual circumstan­ces, strengths and needs.

By providing ongoing non-judgementa­l, compassion­ate person-centred support and understand­ing, we aim to empower people to strive to become the very best version of themselves.
