30 years, but purposely removed from my life as he was always negative and toxic. I still feel bad about it, though. He appears in these dreams, and I wonder if I should go and talk to him, but he never approaches me so I don’t make any effort. In the last dream, my sister (who I think has issues with me) told me that she’d invited him to visit us both. I was livid!

Your instincts are speaking to you through your subconscious to warn you that something definitely is up regarding your relationship with her, and that she has more than a little bit in common with your old toxic ex-friend!
