A spokeswoman for Countrywide, which operates Wallace View, said Cunningham’s employment had been terminated.
“Concerns were identified regarding the management approach of Ms Cunningham while she was in our employment,” the spokeswoman said.
“These concerns were of a safeguarding nature whereby Ms Cunningham failed to safeguard the residents who were in her care at the time.
“Our internal policies were followed appropriately and as a result Ms Cunningham is no longer employed by the company.
“Referrals were made to external agencies as required at the time.”
The regulator also found Cunningham failed to act on a “clear pattern” of behaviour involving the male resident, which could have been addressed appropriately.
