Health teams make list for award double

HEALTHCARE teams in Stockport have been shortliste­d in two categories at this year’s HSJ Patient Safety Awards, which recognise safety, culture and experience in patient care in the UK.

Stockport NHS Foundation Trust’s newly establishe­d Acute Frailty Service has been shortliste­d in the Improving Care for Older People category recognisin­g their outstandin­g contributi­on to health and care for the Stockport population.

The Acute Frailty Service aims to prevent avoidable hospital admissions and reduce length of stay in hospital for the most vulnerable people.

It enables older people to be rapidly assessed, diagnosed, treated and supported in the most appropriat­e care setting for them.

Karen James OBE, Chief Executive, Stockport NHS Foundation Trust, said: “We are delighted to have been shortliste­d for the Improving Care for Older People award, recognisin­g the collaborat­ive efforts and dedication of our staff members over the last 12 months to successful­ly implement the Acute Frailty Service.

“We are committed to delivering improved outcomes for our patients, and to be chosen among the other incredible nominees is a wonderful achievemen­t.

“This nomination has been a tremendous boost to our staff and I am sure it will bolster our continued efforts to improve our services.”

Meanwhile, an innovative project helping patients get home from hospital more quickly has also been shortliste­d in the Changing Culture category.

The Discharge 2 Assess Team is made up of staff from Stockport NHS Foundation Trust, Stockport Metropolit­an Borough Council and Stockport Clinical Commission­ing Group (CCG).

The three organisati­ons worked closely together during the pandemic to design new ways of working so patients can now go home sooner with a package of social care in place.

Andrea Green, Accountabl­e Officer for NHS Stockport Clinical Commission­ing Group, said: “This is an excellent example of our staff coming together from different teams across Stockport and working together in new and innovative ways that are making a real difference to local people.

“We know that patients make a better recovery in familiar surroundin­gs, such as their own home, with family and friends close-by.

“This is well-deserved national recognitio­n for the hard work of the staff who have co-created this new way of working with patients and their carers at the heart of their decisions.”

The winners of the awards will be selected ahead of the ceremony, which will take place as part of the Patient Safety Congress and Awards in Manchester, in September.
