Man, 34, died after doctor failed to read medical notes

A doctor in an overcrowde­d A&E department sent home a patient with a blood clot because he failed to read the second page of his medical notes, an inquest heard.

Alexander Perron, 34, died at home in Fenton, Lincs, in October 2018. He had been admitted to Lincoln County Hospital four days earlier and was diagnosed with a blocked lung artery.

He was due to be transferre­d to a medical emergency assessment unit, but was discharged with antibiotic­s for a chest infection after Dr Mohammed Elsakka, a locum A&E consultant, failed to read all of his medical notes. The inquest heard Dr Elsakka had no more than two hours’ sleep.

Timothy Brennand, the coroner, concluded the death was due to natural causes contribute­d to by neglect.
