Out-of-office errors

SIR – I submitted a Lasting Power of Attorney applicatio­n to the Office of the Public Guardian during the first week of August 2020. I received notificati­on of registrati­on on December 10.

That in itself was a long time, but, unbelievab­ly, the OPG had sent me someone else’s documentat­ion – an appalling breach of security. Following a furious telephone conversati­on I sent the other party’s document back to the OPG and received a letter of apology.

However, my own LPA document had been lost. A letter dated February 1 2021 read: “Unfortunat­ely, it was erroneousl­y dispatched to a third party and has been returned to us.” That document contained the names and addresses of family and friends apart from my own.

It is time for employees to get back into the office, where supervisor­y systems should be in place.
