Nurse pioneers new care model

A north-east nurse is spearheadi­ng a new model of care for patients with longterm conditions.

Kirsteen Coady, an advanced nurse practition­er at Macduff Medical Practice, is pioneering a House of Care framework which will be introduced on August 1.

Mrs Coady was among 21 nurses who started the Queen’s Nursing Institute Scotland (QNIS) developmen­t programme in March after being nominated by her employer.

The charity initiative enables nurses to become catalysts for change within their communitie­s, encouragin­g Mrs Coady to introduce the House of Care model to help Macduff patients with long-term conditions.

The team will introduce care and support plans for each patient with long-term conditions letting them become more involved with their care.

People registered at the practice with a long-term illness will have an initial appointmen­t in their birth month for testing.

They will then be invited to a follow-up meeting to discuss their important issues.

Mrs Coady said: “My hope is that this approach could change the lives of others for the better.”
