Nurse pioneers new care model
A north-east nurse is spearheading a new model of care for patients with longterm conditions.
Kirsteen Coady, an advanced nurse practitioner at Macduff Medical Practice, is pioneering a House of Care framework which will be introduced on August 1.
Mrs Coady was among 21 nurses who started the Queen’s Nursing Institute Scotland (QNIS) development programme in March after being nominated by her employer.
The charity initiative enables nurses to become catalysts for change within their communities, encouraging Mrs Coady to introduce the House of Care model to help Macduff patients with long-term conditions.
The team will introduce care and support plans for each patient with long-term conditions letting them become more involved with their care.
People registered at the practice with a long-term illness will have an initial appointment in their birth month for testing.
They will then be invited to a follow-up meeting to discuss their important issues.
Mrs Coady said: “My hope is that this approach could change the lives of others for the better.”
