Mother’s plea for prison staff forms

A grieving mother has hit out after it emerged key documents were missing that could shed light on why her jailed teenage daughter, who had a history of mental health problems,waslockedu­palone in a cell block with no staff on night duty. Dionne Kennedy, 19, hanged herself with a sheet after being placed in a “normal” cell without regular night-time checks, rather than under observatio­n in a ligature-proof safe room with tear-proof bedding.

A fatal accident inquiry yesterday heard that two forms, which should have given details of how and why staff had found Ms Kennedy was “not at risk” of suicide, had not been found.

The incident happened in October 2014 at Cornton Vale Prison after Ms Kennedy’s final court appearance­s.
