These are summaries of Albuquerqu­e Environmen­tal Health restaurant inspection­s. The best possible score on a restaurant inspection is 214 points. Inspectors subtract points from that total for each violation. A priority violation is the most serious and costs six points (-6) for each individual violation. A priority foundation violation is a medium-risk violation and deducts three points (-3) for each individual violation. A core violation is a low-risk violation and deducts one point (-1) for each individual violation. Restaurant­s with a cumulative inspection score of 80 percent or higher are approved. All inspection­s resulting in a score of 79 percent or less are deemed unsatisfac­tory. Restaurant­s with violations have 10 days to make correction­s. All establishm­ents that pass their inspection­s are listed in the most serious category for which they have a violation.

Downgrade Observed several dead cockroache­s throughout facility. Instructed opera- tor to remove dead cockroache­s. Also instructed operator to fax or email a copy of the past three months’ pest control records to CHPD for verificati­on. Observed facility offers steaks and hamburgers cooked to order without a consumer advisory posted or disclosed. Instructed operator to post consumer advisory reminder. Reminder shall include marking the animal-derived foods requiring disclosure. Observed person in charge (PIC) failed to demonstrat­e duties by receiving several priority violations at the time of inspection. Observed an open package of raw ground beef stored directly on top of a plate of uncovered chile rellenos. Instructed employee to reconfigur­e storage of food items to prevent possible contaminat­ion. Observed employee removing opened package of ground beef. Violation corrected on site. Observed employee utilizing gloves to scoop raw ground beef onto flattop to cook and then proceed to garnish a plate with ready-to-eat shredded lettuce. Instructed employee to dispose of plated food, remove gloves, wash hands and then don fresh gloves. Educated employee on proper glove usage procedures. Observed employee dispose of plated food, remove gloves and wash hands and don new gloves. Violation corrected on site. Observed food debris and build up on the canopener blade. Instructed operator to clean and sanitize can-opener blade after every use. Observed a pan of cooked chicken on the flattop at 121° Fahrenheit. Employee stated that chicken had been there for 20 minutes. Instructed employee to immediatel­y reheat chicken to 165° for 30 seconds. Observed employee reheating chicken at 167° after one minute. Violation corrected on site. Observed dish machine chlorine sanitizer testing at 0 ppm chlorine. Instructed operator to contact repair personnel to have dish machine serviced and to cease utilizing dish machine until it is repaired. Facility must utilize three compartmen­t sink until dish machine has been serviced. Operator must fax a copy of completed work order to CHPD when all repairs have been made to dish machine. Observed service personnel repairing dish machine during inspection. Dish machine retested at 80 ppm chlorine. Violation corrected on site. Observed a leak in the urinal in men’s restroom. Instructed operator to repair leak. Operator must fax a copy of completed work order to CHPD when repairs are made. Observed several holes in the walls and baseboards throughout facility. Instructed operator to repair and seal holes. Observed cracked tiles or missing tiles underneath dish machine. Instructed operator to repair/replace tiles.
