Downgrade

Observed dead roaches inside the unused ice machine. Observed dead roaches underneath the buffet counter. Observed dead roaches in the dry storage area. Observed dead roaches throughout the facility at the time of inspection. Instructed the person in charge (PIC) that the facility must remove all dead roaches and clean and sanitize all areas of the kitchen and any unused equipment that is stored in the kitchen. Observed several food items in the walk-in cooler and reach-in cooler such as cooked rice, noodles, and chicken without any cover. Instructed the PIC that all food must be covered unless cooling. Observed the sanitizer bucket in the kitchen tested at 0 ppm chlorine. Violation corrected on site. Sanitizer bucket retested at 100 ppm. PIC failed to demonstrat­e that employees are informed on reportable illnesses. As corrective action, PIC is to develop a form, policy, and or action plan to inform all employees of the reportable foodborne illnesses and their symptoms. Violation corrected on site by providing two foodborne illnesses and symptoms chart to be posted in the kitchen and front area. Observed several foods stored at room temperatur­e stacked on top of each other without a barrier to prevent contaminat­ion on a counter and food cart at the time of inspection. Observed food such as cooked shrimp temperatur­e at 53.5° Fahrenheit, clams temperatur­e at 51°, ribs temperatur­e at 46.5°, beef temperatur­e at 46.5°, and crab meat temperatur­e at 48°. Violations corrected on site by discarding all the food. Instructed the PIC that all food must be kept at 41° or below for cold and 135° and above for hot holding. Observed several cooked food items in the walkin cooler and reach-in cooler such as cooked eggs, cooked chicken, cooked rice, and cooked beef with no date marking. Instructed the PIC to date mark all cooked foods in the reach-in cooler and walk-in cooler not to exceed more than seven days. Observed both the sanitizer buckets in the kitchen and the front area without a label indicating sanitizer buckets. Instructed the PIC that both sanitizer buckets must be labeled. Observed several containers of a powderlike substance stored underneath the buffet counter and the kitchen area without a proper label indicating what the powdery substance is. Instructed the PIC that when placing any kind of toxic or nontoxic items in a container it must be labeled to indicate what the item is.

Downgrade

Observed the person in charge (PIC) is not ANSI certified at the time of inspection. Observed the facility has priority item violations, priority foundation item violations and core item violations at the time of inspection. Observed the PIC failed to answer or demonstrat­e basic food safety knowledge at the time of inspection. PIC failed to demonstrat­e that employees are informed on reportable illnesses. Corrective action — PIC is to develop a form, policy, or action plan to inform all employees of the reportable foodborne illnesses and their symptoms. Provided a copy of the Form 1B and the five reportable illnesses chart. Observed a cleaner bottle labeled glass spray and light-duty cleaner used as sanitizer at the time of inspection. Instructed the PIC that the facility must have a designated sanitizer bottle or bucket for sanitizer use only and it must be labeled. Facility cannot use labeled cleanser bottle as sanitizer bottle. Observed no thermomete­r inside the reach-in cooler or walk-in cooler at the time of inspection. Instructed the PIC that the facility must obtain thermomete­rs for reachin cooler and walk-in cooler. Observed an open employee drink in a plastic bottle in the kitchen area without a straw. Instructed the PIC that drinks in the kitchen area must prevent hand-to-mouth contact. Observed expired sanitizer test strips and used sanitizer test strips at the time of inspection. Instructed the PIC that the facility must obtain sanitizer test stripes and train all employees and management on how to use the sanitizer test stripes. Observed food employee fail to immediatel­y clean hands and exposed portions of arms after engaging in activities that contaminat­e the hands such as re-entering the kitchen area coming from outside the facility. Violation corrected on site by informing/observing the PIC tell the food employee to properly clean hands before handling ready-toeat foods. Instructed PIC to retrain employees on the requiremen­t of when to wash hands as outlined in section 2-301.14 of the 2009 FDA food code. Observed handwashin­g sink blocked and not accessible in the kitchen area by storing pans inside the handwashin­g sink. Instructed the PIC that the only hand-washing sink in the kitchen must be accessible and available at all times to wash hands.
