Inspection date: Dec. 10 Score: 95- A Violation of Code: Observed overhead AC vents missing covers in several locations throughout food prep area. Corrective Action: PIC was notified. Covers must be installed immediatel­y to prevent possible food contaminat­ion from dust and debris in overhead AC vents. New Violation. Correct By: 12/ 17/ 2014

Violation of Code: Observed food employee with long fingernail­s that extend beyond fingertips. Corrective Action: PIC was notified. Employee was instructed to wash hands and put on gloves while handling food until fingernail­s can be properly trimmed. Corrected OnSite. New Violation.

Violation of Code: No sanitizer buckets set up during inspection. Corrective Action: PIC was notified. Cloth buckets were set up and adjusted to 200 ppm quaternary ammonia as required. Corrected OnSite. New Violation.

Violation of Code: Observed hand sink in restroom not properly caulked to the wall. Corrective Action: PIC was notified. Hand sink will be caulked by next inspection. New Violation.

Violation of Code: Observed mops stored wet in sink basin. No rack provided for hanging mops to dry. Corrective Action: PIC was notified. Mop rack will be purchased and installed. This will be rechecked. New Violation.

Violation of Code: Observed 0 ppm Chlorine at cook line sanitizer bucket. Corrective Action: CFSM was notified of this issue, employee was instructed to refill sanitizer bucket with proper strength solution. Corrected On- Site. New Violation.

Violation of Code: Observed several stacks of pans stacked while wet on clean dish rack ( Wetstackin­g). Corrective Action: CFSM was notified. Employees were assigned to separate dishes for proper air drying. Corrected OnSite. Repeat Violation.

Violation of Code: Observed food buildup on sides of equipment at cook line. Corrective Action: CFSM was notified of this issue, employee was assigned to detail clean. New Violation.

Violation of Code: Observed buildup of trash and food under shelving at walk- in cooler and freezer. Corrective Action: CFSM was notified of this issue, employee was assigned to clean under shelving. New Violation.

Violation of Code: Observed damaged cove base at walk- in cooler floor/ wall juncture. Corrective Action: CFSM was notified of this issue, will place work order to have cove base reinstalle­d as needed. New Violation.
