People living with chronic illness encouraged to sign up for ‘Steps to Healthy Living’ program

Adults, ages 60 or older, who live with chronic health conditions may register for the “Steps to Healthy Living: Chronic Disease Self-Management Program,” sponsored by the Delaware County Office of Services for the Aging, in collaboration with the Delaware County Department of Parks and Recreation. Participants in the program will meet at the Redwood Center in Upland Park in Brookhaven once a week for six weeks, 9:30 a.m. — noon Fridays, May 20 through June 24, with the exception of Friday, June 17, which will be held on Wednesday, June 15, instead.

The program is offered free, through support from COSA and the state Department of Aging and includes a personal, complimentary copy of the book Living a Healthy Life with Chronic Conditions.

The acclaimed, evidencebased program, developed at Stanford University, provides participants with selfcare tools and life skills to support them in reaching their personal goals to manage their chronic conditions. The Centers for Disease Control and Prevention finds that six in ten Americans live with at least one chronic disease and four in ten adults have two or more of these conditions. Illnesses like heart disease, cancer, chronic lung disease, stroke, diabetes, and other chronic diseases are the leading causes of death and disability in America and a leading driver of health care costs.

Space is limited and registration is required. To register, contact Ellen Williams, Health and Wellness Program Manager, at williamse@co.delaware.pa.us or call 610-499-1937.
