Violations/comments: PIC unable to demonstrat­e knowledge by having no critical violations. Observed multiple critical violations at time of inspection. Discussed importance of preventing critical violations to maintain food safety and protect public health. Employees are not maintainin­g properly temperatur­es of TCS foods during hot and cold storage. Critical repeat: Observed hot-holding box and steam well to have foods less than 135 degrees F holding temperatur­es. Food discarded. Observed severe fly and sewer gnat infestatio­n throughout the facility. Kitchen walk-in cooler ambient temperatur­e thermomete­r in disrepair and not reading temperatur­e. Repair thermomete­r. Observed cutting boards in coffee/tea corridor and in prep area to be marred and discolored and not able to be effectivel­y cleaned. Repeat: Observed large amounts of standing water in dish area near three-bay, dishwasher and grease trap. Repeat observed dirty: floor and grouting, kitchen under sinks, bar under sinks, ware washing area in kitchen.

Violations/comments: Corrected during inspection: Critical, hand-washing sink not accessible. Observed hand-washing sink blocked by multiple brooms. Brooms relocated. Critical: Food not properly protected from contaminat­ion by separation, packaging and segregatio­n. Uncovered steaks in reach-in cooler. Steaks were covered. Critical repeat: Equipment food-contact surfaces or utensils are unclean. Observed multiple plates and bowls with food debris near prep line. Plates and bowls moved to dish machine. Observed mold buildup on soda nozzles. Nozzles removed and run through the dish machine. Observed no written procedures for time as a public health control. Equipment components not intact, tight or properly adjusted. Fixed equipment not properly sealed for cleaning. Equipment components not intact. No door handle on microwave. Physical facilities not maintained in good repair. Facility not maintained clean.
