Date of inspection: Nov. 23Violations/comments: Equipment components are not intact, tight or properly adjusted. The gasket on the walk-in freezer was observed in disrepair. Non-food contact surfaces of equipment are unclean. Observed a buildup of grease under the deep fryers. Observed food debris at the bottom of the reach in freezer. Person in charge was very knowledgeable on food safety. Facility was observed orderly.
Bob Evans Restaurant

Violations/comments: Corrected during inspection, Critical Repeat: Food employee(s) did not wash hands when required. Observed an employee change tasks and don new gloves without washing hands prior. Discussed with employee that hand washing must occur when changing tasks and before donning new gloves. Employee proceeded to wash hands. Critical, Equipment food contact surfaces or utensils are unclean. Observed a buildup inside the cappuccino machine. Upon informing an employee, the machine was taken apart and cleaned. Observed employee drinks being stored on prep tables and other equipment throughout the facility. Discussed with the person in charge that personal items should have a designated area. Repeat: observed several working food containers throughout the facility not properly labeled. Repeat: Non-food contact surfaces of equipment unclean. Facility not maintained clean.
Pizza Hut

Violations/comments: Observed magnet strip near dish wash area that was rusted and no longer smooth and easily cleanable. Repair, replace or remove magnet strip. Repeat: Observed nonfood contact surfaces to have grease, dirt and/or food buildup. Clean. Observed flooring to be absorbent and not easily cleanable. Floors need to be addressed so cleanable and non-absorbent or replaced with an approved material. Repeat: Observed physical facilities dirty in need of cleaning, including floor sink under prep sink, floor drain next to dish machine and vent above dish washer. Clean.

Violations/comments: Observed microwave with food debris inside. Clean. Observed broken floor tiles throughout the kitchen. Replace to maintain floor as smooth and easily cleanable. Observed dirt and debris buildup on walls and floor near mop sink, top of great trap in floor near three-compartment sink and flooring in kitchen area. Clean.
