Springmeade Health Center

Address: 4375 S. County Road 25A, Tipp City

Violations/comments: Corrected during inspection, Critical repeat: Employees are not informed in a verifiable manner of their responsibility to report information about their health. A copy of an employee illness reporting policy was given at time of inspection. Person in charge had the employees start signing the policy during the inspection. Corrected during inspection, Critical repeat: The following food-contact surfaces were observed unclean with food debris buildup: can opener, several utensils hanging above prep table, slicer, several dishes on the clean dish rack. All items were removed to the dish area to be cleaned. Equipment components are not intact, tight or properly adjusted. Observed a buildup of ice in both reach-in freezers and the walk-in freezer. Discussed repairing options with person in charge. Non-food contact surfaces of equipment are unclean. Observed buildup of dust on ventilation system in dish area. Repeat: Facility not maintained clean. At time of inspection, the floors and walls in dish area were observed unclean with buildup.

Physical facilities are not maintained in good repair. Observed ceiling tile in front of store in disrepair. PIC stated there has been a work order placed.
