MONTGOMERY CO. HEALTH INSPECTIONS
The following are a sampling of food operation inspection reports from the Public Health – Dayton & Montgomery County, from the week of Feb. 6 to Feb. 10. View full inspections reports at inspections.phdmc.org.
Chiapas
298 N. Main St., Centerville
Date of inspection: Feb. 6 Violations: Observed that the facility does not have an employee with a manager certificate in food protection.
Observed cooked shredded chicken that had been cooked the previous day to be at 93F. PIC discarded of chicken and discussed cooling procedures with PIC.
Observed several refrigerated, TCS foods throughout facility to not be properly date marked. PIC had food date marked.
Observed buildup of debris throughout facility. Observed buildup of debris in outside storage closet.
Comments: Discussed proper cleaning and sanitizing of food-contact surfaces with PIC.
Obtain copy of employee health policy.
Leo’s Mexican Market 748 Troy St., Dayton
Date of inspection: Feb. 8 Violations: Knives being stored between the wall and a conduit line in the kitchen area. Ensure knives are stored in contact with clean surfaces between uses.
Employee in the kitchen area rinsed off a cloth in running water to wipe down a preparation surface. Ensure that surfaces are being sanitized by keeping cloths in sanitizer of proper concentration or other proper means to sanitize food-contact surfaces.
Discussed with the person in charge and the employee using the damp cloth during the inspection.
No ingredient labels for the self-service doughnuts near the front door. Ensue ingredient information is available for patrons to review.
No soap for the hand sink in the meat cutting area. Ensure hand sink is properly provisioned for employees to wash hands.
Obtained soap for the hand sink and corrected.
Comments: Rice cooling at the time of the inspection. Rice at 150F on the countertop at the time of the inspection. Put into plastic bags and place into the walk-in refrigerator to complete the cooling process. Reheat for hot holding. Process meats minimum temperatures needed.
Wendy’s
2311 Needmore Road, Dayton
Date of inspection: Feb. 8 Violations: Observed cheese sauce (74F) hot holding below 135F at prep line. PIC stated cheese sauce was recently prepared and placed in microwave to heat to proper temperature.
Observed buildup in facility (sides of fry/cook equipment, floor underneath cook line, inside refrigerator below bulk lemonade, hard-to-reach areas, etc.). Clean and maintain.
Observed chemical spray bottle stored in food area. PIC removed bottle and placed in chemical storage area.
Comments: PIC stated managerial staff members are new and haven’t completed Managerial/Level 2 course yet. Ensure course is completed to obtain Ohio Department of Health certificate.
Hardees
7508 Brandt Pike, Huber Heights
Date of inspection: Feb. 6 Violations: This department received a complaint about unsanitary practices allegedly occurring on 2/5/23.
At time of inspection, observed one of two employees not wearing hair restraints. PIC provided employee with a visor to be worn while working.
Observed one employee in a walking boot, no flip flops observed. PIC stated they are not aware of any employees wearing flip flops while working.
Observed gloves in use at time of inspection.
Observed paper towels available at hand sink.
Discussed employee cleanliness with PIC.
No follow-up required.
