OBSERVED: (1) At least one EMPLOYEE that has supervisory and management responsibility and the authority to direct and control FOOD preparation and service shall be a certified FOOD protection manager (CFM) who has shown proficiency of required information through passing a test that is part of an ACCREDITED PROGRAM. • CALION MIDWAY MART INC DBA QUI, 7911 CALION, AR. OBSERVED: (1) The facility does not yet have a CFM. At least one EMPLOYEE that has supervisory and management responsibility and the authority to direct and control FOOD preparation and service shall be a certified FOOD protection manager (CFM) who has shown proficiency of required information through passing a test that is part of an ACCREDITED PROGRAM. (2) Deli items in freezer were left uncovered. CORRECTIVE ACTION TAKEN

(2) Foods such as kolaches, sausage, egg, and bacon were out at room temperature with no time holding documented. (3) A case of bacon was left out and unrefrigerated. CORRECTIVE ACTIONS TAKEN
