How to enroll parents

If you are enrolling your parents with Medicare, you will need a couple of additional things.

You will need a form called “Authorization to Disclose Personal Information” to register a parent. It is also important to have a discussion with your parent on their medical needs.

You can find this form on Medicare's website, medicare.gov. With COVID-19 cases continuing in United States, it is a good time to enroll your parent in Medicare coverage.
