Our senior care puts the focus where it belongs: on you. Your primary care provider (PCP) acts as your guide and partner. Your PCP will help you with specialty care referrals, medication­s, tests and treatments to keep you well.

Keep up with your health

We want to make sure you don’t feel rushed and have time to discuss any issues or ask any questions, so we can get to know you and your medical history -- that’s why we’re proud to offer longer first visits with your annual wellness visit. This extended visit is a full comprehens­ive, one-hour physical exam. It will give you the opportunit­y to get answers to all your questions. This exam also provides the opportunit­y to update your medical history. As part of the exam, we will review your medication­s and you may receive tests based on your individual needs.

Personaliz­ed care

We provide custom care for your unique health needs. Our care offerings include hundreds of health care providers, primary and preventive care to keep you healthy, flu and pneumonia shots, bone mineral density tests if needed, diagnostic services (EKGS), lab services (blood tests, urine tests, etc.) and radiology services (digital X-rays). Southwest Medical also offers priority access for our senior patients. Program highlights include direct access to the Senior Care Unit where you can make a same day appointmen­t and get personaliz­ed access to your care team of providers.

Telephone Advice Nurse Line

Our Telephone Advice Nurse Line allows you to speak with a nurse 24 hours a day, seven days a week. If you’ve visited one of our locations, the nurse can access your records.

Optum Care Community Centers

Our community centers are a great way for people ages 55+ to stay connected, informed and fit. Creating a healthy lifestyle is simpler when you’re not doing it on your own. We offer dozens of ways to help you get moving and enjoy. Older adults can laugh, learn and stay healthy with weekly programs and activities or just meet up with friends.

Want to learn about Medicare, computers, the latest health news, cooking and more? Give our no-cost classes a try. We offer all kinds of activities, from dancing, fitness and yoga classes to bingo, card games and support groups.
