A Tribeca event planner, her pilot boyfriend and her two young sons disappeared in a plane in the Bermuda Triangle, and authorities said Tuesday they found a “debris field” near where the aircraft was last heard from.

The group had just spent Mother’s Day in Puerto Rico and was flying to Florida when the small twin-engine plane disappeared at about 2:10 p.m. Monday, according to officials.

On board were 40-year-old Skylight CEO Jennifer Blumin, her 4and 3-year-old sons and her boyfriend, Nathan Ulrich, 52, of Lee, NH, authorities and family members said.

Ulrich, an Ivy-League-educated former Coast Guard auxiliarist, is the ex-husband of actress Rae Dawn Chong.

Later Tuesday, the Coast Guard said the debris field appeared to be from the missing plane and the search for survivors was continuing.

Blumin’s ex, the boys’ father, is James Ramsey, a top Manhattan designer behind a planned Lower East Side underground park dubbed the Lowline.

“We’re waiting for the Coast Guard to tell us whether our grandchildren and our children are alive,’’ said a distraught man answering the phone at the home of Blumin’s parents in Tarrytown, Westchester.

“We’re waiting with bated breath for the Coast Guard to call.”

The plane, owned by Blumin, took off from Rafael Hernandez Airport in Aguadilla, PR, at 11:08 a.m. Monday, officials said.

Ulrich’s last Instagram posting was from an organic Puerto Rican restaurant the day before — on Mother’s Day.

“Mother of a day @cartabuena #rincon,’’ his post reads.

The plane was supposed to land for refueling at the Space Coast Regional Airport in Titusville, Fla., at 2:44 p.m., according to aviation records.

But about a half-hour before its scheduled landing, the plane disappeared from the radar of Miami air-traffic controllers.

The MU-2B aircraft was at 24,000 feet and traveling 345 mph when it vanished 37 miles east of the island of Eleuthera thera in the Bahamas, officials said.aid.

“There’s no indicationtion of signif-significant adverse weatherther at the time,” said Coast Guard spokesman Lt. Cmdr. Ryan Kelly.

The Coast Guard tweeted Tuesday afternoon that one of its helicopters located a “debris field approx. 15 mi E of Eleuthera, Bahamas.”

An aircraft seat was among the floating debris, said Coast Guard spokesman Eric Woodall.

“They recovered a few things to ID [the plane] and help the investigation,” Woodall said. The chopper lowered a would-be rescuer to grab the items.

“Search for survivors continues,” the Coast Guard tweeted.

Woodall added, “We’re search- ing as long as we think we can find them alive.”

Relatives of the missing were numb.numb.

“Right now we’re still dealing with the uncertainty,” UIrich’s father told The Post through tears.

“We have nothing other than they’re still searching.”

Chong, the daughter of Tommy Chong of Cheech & Chong fame, tweeted, “My ex-hubby’s (Nathan Ulrich) plane is missing over the Bermuda Triangle. I am sad and in shock. So send prayers to his family. Thanks.”

She told The Post, “We’re all in massive shock.

“He’s a really, really, really good pilot,’’ she said. “For him to be missing, that’s not good.’’

The pilot’s brother, Walter

Ulrich, 48, of Manhattan, said he’s trying to remain optimistic. “I hope he’s treading wa-water somewhere in the Caribbean,’’ Walter said. “He’s an experienced pilot.” Still, “it doesn’t look good right now.”

Blumin is the owner of Skylight Studios, an ultra-hip event producer that caters to the film and fashion industries.

She owns a home on White Street, down the block from the design studio of her ex, Ramsey. The home went up for sale last week.

Neighbors at a property Blumin owns in Southold, LI, said she purchased the stunning waterfront home last year.

“Oh, God, it’s just terrible, just awful,’’ said next-door neighbor Sam Wilkenson, 88. “This was goinging to be their first full summer here,’’ he said of Blumin and her children. “Those little kids. And she’s a young woman! Terrible! Just terrible.”

According to a 2105 New York Times story, Blumin’s and Ramsey’s older son is named Phineas; the younger son is Theodore.

A source said Ramsey “was already so depressed about the breakup” with Blumin.

Ulrich’s ex, Chong, said she met Blumin through him.

“I love him,’’ she said of Ulrich. “He’s a really kind person, and [Blumin] is an adorable woman, and the kids are great.
