Restaurant inspections

Critical violations are those factors leading to foodborne illness and must be corrected immediately. Noncritical violations relate to maintenance of food operations and cleanliness.

BENTON COUNTY Feb. 6 Cadence Academy Preschool

Popeyes

3500 U.S. 412 East, Siloam Springs

Critical violations: Employee did not wash hands between task change and putting on clean gloves. Several dishes (lids, pans, pan drainage trays) on the clean dish storage rack are visibly dirty with particles of food. The drainage trays that go in the bottom of a pan are also greasy to the touch. Manager said time is used as a primary control for chicken. No time indicated on items on the line at the beginning of the inspection. Portioned rice on the serving line 119 to 124 degrees. Manager said the portioned items have a specified holding time, yet no time was marked or indicated in anyway.

Noncritical violations: Sanitizer water has food debris in it and is cloudy. Food debris and flour residual on the plastic curtains in the door of the walk-in cooler and on the edge of the door. Food debris (not just flour) under the shelves in the dry storage area. Areas under the stationary equipment on the make line (not chicken fry station) are not clean.

Pure Joy Ice Cream

Critical violations: None Noncritical violations: Food workers shall wear hair restraints, beard restraints. Clean dishes/utensils in direct contact with cloth towel to air dry. Posted retail food permit has expired and not in customer view.

Feb. 7 Burrito House and Taqueria

183 Rebecca St., Suite C, Gentry Critical violations: Demonstration of knowledge of food safety was not evident throughout the facility. Employee handled a tortilla, lettuce, pico and wrapped up a burrito with bare hands. Handwashing sink is dirty and does not look like it has been used. Open package of chicken and bag of beef stored over ready to eat foods. Portioned cups of queso sitting on top of a pan with a lid in the hot well. Container of rice and container of cooked chicken not at 41 degrees or below. According to employee, chicken was sitting out during lunch service to portion and did not get put away until after lunch. Rice may not have gotten cooled properly or the storage container is not appropriate to use to keep food cold (plastic too thick to prevent air conductivity). No date marking in foods as needed. Spray bottles not labeled with contents. Ice scoop stored laying down in the ice.

200 First Ave., S.E., Gravette Critical violations: Hamburger meat in fresh cold hold out of date 2/6/20.

Milk in dairy fridge out of date Jan. 31, Feb. 3, Feb. 5 and Feb. 6. Fresh eggs marked Jan. 31. Utensils in meat department in the gap between the wall and the three-compartment sink. Raw meat and debris on the floor, leg of table and around the plastic wrapping machine.

183 Rebecca St., Gentry Critical violations: Portioned meats stored above the load line are not holding at 41 degrees or below. Teriyaki chicken 43 degrees, chicken strips 44 degrees. Food checked in the reach-in cooler is above 41 degrees.

Noncritical violations: Thermometer in reach-in cooler is broken. Water leaking under and around the bag in a box station. Leak under the beverage station. Open packages of lids had liquid leaking on them.

1387 S. 48th St., Springdale Critical violations: Repeat violation-the handwashing sink in the dish area is covered with a purse. Sliced oranges stored below chopped raw meat products. No sanitation water was available for sanitizing the food contact surfaces of the kitchen. Three-compartment sink being used for washing serving platters; there was no sanitation in the third-compartment sink.

Noncritical violations: Trash cans in the kitchen have a buildup of soils, and also, the areas around the grill and under have a buildup of food residues.

All foods shall be kept cold at 41 degrees or below.

Noncritical violations: Temperature of water where utensils are stored between uses is at 89 degrees. Walk-in-cooler shelves are covered with cardboard. Interior of ice machine and can opener (has food debris) are not clean. Table fan is not clean.
