3500 U.S. 412 East, Siloam Springs

Critical violations: Employee did not wash hands between task change and putting on clean gloves. Several dishes (lids, pans, pan drainage trays) on the clean dish storage rack are visibly dirty with particles of food. The drainage trays that go in the bottom of a pan are also greasy to the touch. Manager said time is used as a primary control for chicken. No time indicated on items on the line at the beginning of the inspection. Portioned rice on the serving line 119 to 124 degrees. Manager said the portioned items have a specified holding time, yet no time was marked or indicated in anyway.

Noncritica­l violations: Sanitizer water has food debris in it and is cloudy. Food debris and flour residual on the plastic curtains in the door of the walk-in cooler and on the edge of the door. Food debris (not just flour) under the shelves in the dry storage area. Areas under the stationary equipment on the make line (not chicken fry station) are not clean.

Feb. 7 Burrito House and Taqueria

183 Rebecca St., Suite C, Gentry Critical violations: Demonstrat­ion of knowledge of food safety was not evident throughout the facility. Employee handled a tortilla, lettuce, pico and wrapped up a burrito with bare hands. Handwashin­g sink is dirty and does not look like it has been used. Open package of chicken and bag of beef stored over ready to eat foods. Portioned cups of queso sitting on top of a pan with a lid in the hot well. Container of rice and container of cooked chicken not at 41 degrees or below. According to employee, chicken was sitting out during lunch service to portion and did not get put away until after lunch. Rice may not have gotten cooled properly or the storage container is not appropriat­e to use to keep food cold (plastic too thick to prevent air conductivi­ty). No date marking in foods as needed. Spray bottles not labeled with contents. Ice scoop stored laying down in the ice.
