Restaurant inspections

Critical violations are those factors leading to food-borne illness and must be corrected immediately. Noncritical violations relate to maintenance of food operations and cleanliness.

BENTON COUNTY Nov. 23 Havana Tropical Grill

203 N. Second St., Rogers Critical violations: Ground beef in steam table being held at 79 degrees and should be held at 135 degrees or above. Multiple food items out of date in walk-in cooler.

Noncritical violations: Chicken thawing on open air counter top. Mesh bags of onions and cardboard boxes of food items being stored directly on floor of dry storage and walk-in freezer.

Noncritical violations: Ventilation fans required in the restroom facilities.

2723 U.S. 412 East, Siloam Springs Critical violations: Automatic dishwashing machine is not reaching the proper sanitizing temperature. Violation repeated multiple times. Noncritical violations: No date marking on the container of chili in the back refrigerator. Cabinet under both soda machine units is not clean. Spilled liquid under each unit. Excessive ice buildup on freezer unit and the freezer door and frame. Evidence of water dripping onto the product cases. Repeat violation. Posted retail food permit has expired.

190 Progress Ave., Siloam Springs Critical violations: Bone-in chicken wings not at 135 degrees or above. Multiple food items on cold line not at 41 degrees or below. Noncritical violations: No thermometer in cold holding unit. Excessive grease and dirt buildup in the area off the back door/exterior mop sink area. The sidewalk out the back door is black with a buildup of grease and dirt and into the parking lot. Posted retail food permit has expired.

Critical violations: Multiple food items temperatures in the walk-in cooler are inconsistent of the required 41 degrees or below.

300 E. Centerton Blvd., Centerton Critical violations: Open lid drink above cold hold. Refried beans, peppers, lettuce, tomatoes and onions uncovered in the walk-in cooler. Plastic glass containing cleaning solution unmarked on a prep table. Rice, refried beans, carne and chicken in the hot hold area uncovered. Used wiping cloth left on cashier’s counter.

Noncritical violations: Top of ice machine has a layer of dust and other accumulation. Multiple seats torn and showing stuffing material. Plaster falling off the walls at one booth.

El Pueblo

1549 W. Hudson Road, Rogers Critical violations: No paper towels available at kitchen handsink at time of inspection. Sanitizer bucket greater than 200 ppm.

Noncritical violations: Shelving holding food items and mixer made of bare wood.

Petit Bistro

2702 N. Walton Blvd., Bentonville Critical violations: None Noncritical violations: Wood holding up shelves in food prep area does not have a smooth and easy to clean finish.

Noncritical violations: There is an accumulation of food crumbs on the shelf above the cooler in the sandwich make area. Top of hood above fryers has an accumulation of food crumbs and debris. Floor around soda boxes, ice machine and behind cooler has an accumulation of food debris, crumbs and dirt.

Critical violations: No paper towels were available at the handwash sink. Chlorine in dish machine at 0 ppm on chlorine test strip. Chicken dish in hot hold unit was at 109 degrees and should be at 135 degrees or above. Multiple containers of food in walk-in cooler did not have a date mark. Noncritical violations: Tamales, 118 degrees in large cooking pot and should be at 135 degrees or use within four hours of cooking.

Center Point Convenience Store 1675 W. Centerton Blvd., Centerton Critical violations: Corn dogs, pepperoni pizza in hot hold cabinet was at 126 degrees and should be at 135 degrees or above in hot holding. Noncritical violations: Used wiping cloths on the counter. Nonfood contact surfaces, floor, light switch and door handles surface contaminants.

Harps Deli-Bakery

710 W. Centerton Blvd., Centerton Critical violations: Handwash sink in back not accessible, no disposable towels. Handwash sink in front, no soap, towel or ADH signage. All to be replaced as soon as possible and employees must use another sink with supplies until then. Noncritical violations: Deli freezer unit leaking, ice building up on unit.

Harps Food Store

710 W. Centerton Blvd., Centerton Critical violations: Food in the main vegetable produce fridge 44-45 degrees and should be at 41 degrees or below. Main freezer has a large amount of ice buildup on the compressor unit. Additionally, the door has jagged metal and the seal is missing in places causing ice and water slippage hazard.

Noncritical violations: Used wiping clothes left on table next to food.

Kidz In Motion Learning Center

211 Commercial St., Centerton Critical violations: A handwashing sink shall be maintained so that it is accessible at all times for employee use, and do not use for purposes other than handwashing. Milk temping at 48 degrees in small fridge. Noncritical violations: Masks not being worn by two employees.

Pho Thanh Restaurant

1204 S. Walton Blvd., Bentonville Critical violations: Raw eggs stored above onions in walk-in cooler. Brown gravy at 133 degrees in crock pot and should be at 135 degrees or above. Containers of broth cooling on walk-in freezer floor.

Noncritical violations: Tape is being used to hold up a sheet of metal behind meat slicer.
