Shrimp and beef were being thawed in the three-compartment sink at room temperature without running water. The door to the exterior dry storage area was slightly open to allow a charging cord to run from outlet to EV. The dumpster lid was open. A cell phone was on the cutting board of the prep table.

An employee washed hands with gloves on in the three-compartment sink that is immediately adjacent to a food prep table. This exposed the sushi he was preparing and the food contact surface to potential chemical contamination of soap due to splash.
