Priority foundation violations: Employee uncovered beverage was on the preparatio­n table next to open food. Food and what looked like beverages being dumped down the handwashin­g sink. Sink was not clean. Clean dishes were stored in crates on the drain boards of the three-compartmen­t sinks were getting dirty again with splash/sprays while washing dishes. Dirty dishes were stacked high in the sink and coming into contact with clean dishes. Clean dishes were stored on visibly dirty drain boards. There was a gap in the bottom of the back exterior door. The gap was large enough to allow insects or rodents access to the facility. Beverage bottle was stored in the ice used for beverages. Ice scoop was stored with handle down in the ice bin. Clean containers used for food storage were stored on the floor in the kitchen area.
